BY SIGNING THIS APPLICATION, THE APPLICANT AGREES:

• If selected, vendors must sign an event agreement, provide a Certificate of Insurance and all required licensing as per the event agreement from Hennepin County & the City of Maple Grove. 
• Vendors are responsible for keeping the area around their truck/booth free of garbage and debris. 
• This is an outdoor festival which will take place rain or shine.
• Event fees and application fees are non-refundable.
• Completed application is not a guarantee of acceptance. 
• This application is in no way binding to the Maple Grove Business Association. 
• Your signature below indicates that you have read the rules and regulations and that the information you have provided on this application are true and correct.
